Instead, you pay a predictable monthly fee for a care experience designed around longer visits, continuity, and more personal physician attention.If you have insurance, you can generally continue using it for eligible outside services such as specialist visits, imaging, prescriptions, and hospital care, according to your plan’s benefits.
Insurance can be valuable for hospital care, specialist services, and other medical expenses. Your Mozaïk membership is an investment in something different: a primary care relationship with more time, continuity, and personalized attention built into it.Instead of waiting until something is wrong or trying to address every concern in a brief appointment, you have a physician who knows your history, helps you stay on top of your health, and works with you on a plan that fits your life.For many members, that relationship is the value they have been missing from their healthcare.
Every membership begins with a comprehensive two-hour intake, giving your physician time to understand your medical history, current concerns, family history, lifestyle, and goals.Your membership also includes three quarterly follow-ups, annual foundational labs, ongoing primary care, and acute care access according to your tier. Depending on your membership, you may also receive extended visits, targeted Health Map testing, body composition tracking, and additional physician access.The goal is not simply to give you more appointments. It is to give you a consistent physician relationship and a care plan that can evolve with you over time.
Rooted Care is for someone who wants a trusted primary care home, thoughtful prevention, and a physician who knows their history and helps them stay on top of their health.Vitality Care, our Signature Membership, is for someone who wants to understand their health more deeply. It includes extended visits and a targeted Health Map assessment to help explore concerns such as energy, metabolism, hormones, weight, or other ongoing changes—and build a more personalized plan.Legacy Care is our highest-touch partnership for someone who values greater access, care coordination, and a longer-term approach to their personal and family health.If you’re considering Rooted or Vitality, a complimentary Fit Call can help you decide. Legacy follows a separate inquiry process.

You have a physician who takes the time to understand your health history, listen to your concerns, and help you connect the dots between how you feel, your medical findings, and the life you actually live.Our intentionally small patient panel allows us to offer longer visits, greater continuity, and more personal attention. Instead of starting over with a different provider or trying to fit everything into a rushed appointment, you have a consistent physician partner who knows your story and helps you make informed decisions about your health.
Direct Primary Care, or DPC, is a membership model in which you pay your physician directly for the primary care services included in your membership.At Mozaïk, this allows us to structure care around longer visits, continuity, and a more personal physician-patient relationship. Your monthly fee covers the services included in your tier, so you know what to expect and can focus on your health rather than the cost of each individual visit.
Your membership already includes the laboratory testing specified in your tier. If additional testing is recommended, eligible outside services may be covered by your insurance depending on your plan.Your physician will explain why additional testing may be helpful and discuss any associated costs before ordering.
You can join Mozaïk with or without health insurance.Your membership provides your primary care relationship, while many members maintain insurance for hospital care, emergencies, specialist services, and other medical expenses outside the practice. Mozaïk is not a replacement for comprehensive health insurance.
Mozaïk is here for more than scheduled wellness visits. Your membership includes acute care access according to your tier, so you have a familiar physician to turn to when an illness or concern arises.Mozaïk does not provide emergency or after-hours acute coverage. For emergencies, call 911 or go to the nearest emergency department.
Absolutely. Mozaïk is designed to be your primary care home for preventive care, routine and chronic condition management, acute illnesses within our scope, and ongoing lifestyle medicine guidance.Whether you are managing high blood pressure, diabetes, metabolic concerns, or simply want to stay on top of your health, your physician can help you understand the full picture and coordinate appropriate next steps when specialist care is needed.
Yes. We welcome patients from throughout Georgia who can visit our Smyrna office for their initial and periodic in-person care.Virtual visits may also be available when appropriate, allowing you to maintain your relationship with your physician between in-person appointments.Telehealth in Florida is coming soon, join our newsletter to receive updates and join our Florida waiting list.

The Health Map™ is our physician-guided process for bringing your symptoms, medical and family history, lab results, body composition, lifestyle, and goals into one clearer picture of your health.Rather than looking at individual numbers in isolation, your physician uses this information to identify what deserves attention, understand possible contributing factors, and build a personalized plan for what comes next.Vitality includes one annual assessment with the Foundational Lab Panel plus one targeted pathway selected with your physician. Legacy includes biannual assessments to help track relevant changes over time.
Vitality is also for people who want a deeper understanding of their health, a more individualized prevention strategy, or additional time to explore their health goals.You may have ongoing concerns you want to investigate, a strong family history of certain conditions, or simply want a more detailed picture of your health. Your physician will help determine which testing and strategies are appropriate for you.
Legacy is also for someone who values greater physician access, more comprehensive care coordination, and a long-term approach to their health.It is designed for people who see their health as part of the life and legacy they are building, including understanding family health history, identifying risks, and making informed choices that may benefit both their own wellbeing and the generations that follow.
Not exactly. Concierge medicine typically charges a membership fee on top of billing your insurance.
Direct Primary Care replaces insurance billing for primary care entirely, making the model simpler, more transparent, and focused entirely on you.
