The Rooted Care Membership is MOZAÏK's primary care membership, a Direct Primary Care model that gives you time, access, and a physician who actually knows you. Based in Smyrna, GA and serving adults across Metro Atlanta.
You pay one flat monthly fee directly for your primary care. No co-pays. No insurance billing for our services. No compromises on your time.
Your insurance still covers labs, imaging, specialists, prescriptions, and emergency care. MOZAÏK remains your primary care home.

Reserved for members who join before April 1, 2026. After that, membership opens at $250/month.
Founding members are grandfathered at $175 for as long as their membership stays active.

MOZAÏK is intentionally small. A limited patient panel means your physician has reviewed your history before you walk in, has time to think through your care plan, and is fully present during your visit.

MOZAÏK operates on a Direct Primary Care model. Your monthly membership covers the doctor-patient relationship directly, removing insurance from routine primary care. Your existing insurance still works for labs, imaging, prescriptions, specialist referrals, and hospital care.
We Speak English, French, and Haitian Creole.
MOZAÏK is not an emergency or urgent care facility. For emergencies, call 911 or visit your nearest emergency room.
Mozaïk provides complete primary care. We do not offer onsite labs or imaging at this time. When you need these, we happily refer you to our trusted network of local specialists in the Atlanta area.
Mozaïk is out-of-network by design and doesn't accept insurance, Medicare, or Medicaid. This direct-pay model lets us provide care that's personal, unhurried, and free from insurance limits.
We provide paperwork for possible out-of-network reimbursement (approval is dictated by your insurance plan). Many patients use HSA or FSA funds for our services.
All memberships require a 6-month commitment to ensure we can build a real relationship and provide consistent care.
